Call for Nominations for We Honor Veterans Awards
Submit nominations by September 16, 2016

August 15, 2016 — Care Dimensions, formerly Hospice of the North Shore & Greater Boston, invites the community to nominate a person or persons who has shown commitment to improving the lives of veterans for their annual We Honor Veterans recognition events to be held in November.  As a Level Four Partner of the We Honor Veterans program, Care Dimensions has joined together with the U.S. Department of Veterans’ Affairs (VA) and the National Hospice and Palliative Care Organization (NHPCO) to improve access and to provide the highest quality end-of-life care for our veterans. We recognize there are many individuals in organizations or businesses who through their own efforts improve the lives of veterans every day and we want to recognize these individuals for their commitment. Nominees can be anyone in the community – a private individual, a business owner, a volunteer, a veteran or civilian. People can submit their nominee’s names, along with a 300 words or less telling why they are making the nomination. Please submit all nominations no later than Friday, September 16, 2016 to Sheryl Meehan at smeehan@CareDimensions.org or mail to Care Dimensions, We Honor Veterans Award, 75 Sylvan Street, Suite B-102, Danvers, MA 01923. For more information visit www.CareDimensions.org/veterans.
