Dementia GUIDE Program

New Dementia Support Available

If you or a loved one have been diagnosed with mild, moderate, or severe dementia, you may be eligible for support through the federal government’s new program, Guiding an Improved Dementia Experience (GUIDE).

Care Dimensions has been selected by the Centers for Medicare and Medicaid Services to provide GUIDE services that aim to:

  • Improve quality of life for people living with dementia
  • Reduce strain on their unpaid caregivers
  • Enable people living with dementia to remain in their homes and communities as long as possible

Program highlights

  • Services are covered under Medicare at no additional cost to the patient and caregiver.
  • Comprehensive assessment and care plan for the person with dementia
  • Care coordination with the individual’s health care providers – You do NOT need to change your doctor to participate in the program.
  • A trained care navigator who will help connect the individual and caregiver to community-based resources and supports
  • Caregiver services, including training on how best to care for a loved one living with dementia, education, and 24/7 access to a support line

Eligibility

A person is eligible to receive services under the GUIDE program if they meet the following criteria, as verified by Medicare:

  • Has mild, moderate, or severe dementia, as confirmed by one or our Care Dimensions physicians or nurse practitioners
  • Is enrolled in Medicare Parts A and B (not enrolled in Medicare Advantage, including Special Needs Plans, or PACE programs) and has Medicare as their primary payer
  • Is not enrolled in hospice
  • Is not a long-term nursing home resident
