Director of Quality Improvement and Patient Safety RN


​Summary:

Qualifications: 
 

  • Bachelor’s degree and licensed Registered Nurse required; master’s degree in a healthcare discipline and experience in home care and/or hospice preferred

  • Minimum five years progressively responsible experience, including patient safety/risk management, quality improvement and management experience in a healthcare organization. Experience with facilitation of systems thinking activities such as Root Cause Analyses

  • Demonstrated expertise in the theory and principles of quality improvement, including the use of data to drive performance, safety culture and response to error, and in successfully implementing them in a healthcare delivery setting. Certified Professional in Healthcare Quality designation preferred 

  • Thorough knowledge of hospice regulatory requirements and accreditation standards (DPH, Medicare, JCAHO and/or CHAP), experience with licensure and accreditation surveys and reporting to external agencies

  • Demonstrated knowledge of workplace/patient safety and infection control principles

  • Demonstrated ability to analyze, track and trend quality, safety and performance data
