Evening Nurse, Monday - Friday; 4 p.m. - 12:30 a.m.
The Evening Nurse works under the supervision of the Field Evening/Night Clinical Manager or designee during scheduled Evening hours, Monday through Friday after the close of regular business hours, from 4:00 p.m. until 12:30 a.m.. The Evening Nurse carries a cell phone, responds to visits scheduled; promptly by telephone to requests, and makes home and facility visits as indicated by patient or family need.
Is available evenings during the assigned hours for all Care Dimensions patients, making visits as the patients’ conditions warrant. Must be at the patient’s home or facility as scheduled, and within one hour of call.
Provides quality professional nursing care to patients based on the Plan of Treatment with regard to the physical, emotional and psychological needs of the patient and their families.
Collects data from the patient, family and other health care providers and community agencies.
Contributes to the written interdisciplinary Plan of Care based on nursing assessment data and physician orders.
Documents on a timely basis, the patient’s condition and all care provided using acceptable charting format, following appropriate regulatory/Medicare guidelines.
Ensures privacy for the patient and maintains confidentiality of condition and/or other patient information.
Operates as a team player by approaching relationships in a manner that avoids antagonism and reduces conflict.
Coordinates effectively with all other personnel, agencies and health care providers involved with the patient.
Make routine visits, as directed, to facilitate plan of care and assist with appropriate interventions.
