Thank you for your interest in being a volunteer at Camp Stepping Stones. Please read this application carefully and complete all sections. Each volunteer must complete a required Camp Stepping Stones training. Children ages 12 to 15 years of age may volunteer with a parent or guardian who will participate in both the training and volunteer at camp.

I, , give permission for my child, , to volunteer at Camp Stepping Stones, a bereavement summer camp sponsored by Care Dimensions. If accepted as a volunteer, I understand that my child will be provided with the training necessary for the safe and responsible performance of his/her assigned tasks. *
I understand that I am required to also volunteer at Camp Stepping Stones and I agree to accompany my child to a volunteer training session. I will support my child by respecting his/her volunteer commitment and providing transportation to/from the training session and Camp day. I understand that my child and I might be assigned to different tasks/activities at Camp depending on the needs.*
As part of this volunteer experience, I understand that my child might be photographed or filmed. I hereby grant Care Dimensions permission to use photographs and/or video footage of my child, or statements made by him/her, in any publicity, advertising, website or other similar materials.
