Kristin Chouinard, MSN, AGNP-C, is a nurse practitioner with the Care Dimensions HomeMD primary care program. She has been providing primary care to patients at home with Dr. Messenger since 2018. Previously, she worked as a wellness nurse for Partners HealthCare at Home and was a hospice admission liaison nurse for Care Dimensions (then known as Hospice of the North Shore and Greater Boston). She is board certified as an adult/gerontological nurse practitioner and has a master’s degree in nursing from Regis College and a bachelor’s degree in nursing from Salem State University. Additionally, she has advanced certification in hospice and palliative care nursing.
