Jason Koch, DO, HMDC

Hospice and Palliative Care Physician

Dr. Koch joined Care Dimensions in early 2023 as a full-time hospice and palliative care physician providing care to patients on the North Shore and to those at the Kaplan Family Hospice House in Danvers.

Previously, Dr. Koch served for four years as a palliative care physician with Foundation Medical Partners in Nashua, NH, and provided inpatient palliative care referral support and follow-ups at Southern New Hampshire Medical Center. 

Dr. Koch is certified in family medicine, hospice and palliative medicine, and as a hospice medical director. He received his Doctor of Osteopathic Medicine degree from the Philadelphia College of Osteopathic Medicine. He did a three-year residency in family medicine at Christiana Care Health System in Wilmington, DE, where he followed with a one-year hospice and palliative medicine fellowship.
