Advanced Illness Support

Daily life can be increasingly difficult for people living with chronic or advanced illness — and a constant challenge for their caregivers. Our private-pay Care Connections Program helps by adding a layer of expert assistance for any patient with any chronic or advanced illness who is not enrolled in hospice care. Through personalized care coordination that focuses on symptom and pain management, patients regain independence and enjoy a better quality of life.

Expert Assistance

Our care begins with an in-home medical screening by your nurse case manager, who will:

  • Review medications
  • Identify potential issues with your illness
  • Conduct a home safety screening
  • Develop a customized care management plan
  • Check in with you regularly by phone to provide or coordinate services
Multidimensional Care

Our Care Connections program provides multiple services, including:

  • Pain and symptom management
  • 24/7 telephone support
  • Assistance managing medications and refills
  • Advice on whether symptoms require a doctor’s appointment
  • Monthly visits from your nurse case manager
  • Visits from trained volunteers for companionship, errands or transportation
  • Referrals to community resources to meet your needs
  • Education on your illness and symptom management
  • Regular updates for your caregivers and family members, if requested

Additional nursing visits can be added for a fee.

The $350 monthly fee covers all services plus the comfort of knowing you have access to a registered nurse by phone 24-hours a day. This extra layer of support can help you:
