American Heart Association Heart Failure Certification

Care Dimensions is the first hospice and palliative care agency in the country to earn Palliative/Hospice Heart Failure certification from the American Heart Association®(AHA). The certification provides an evidence-based framework for evaluating palliative and hospice care agencies against the AHA’s rigorous science-based standards for heart failure patients.

Care Dimensions is Accredited by CHAP

Since 2001, Care Dimensions has been accredited by The Community Health Accreditation Program (CHAP). CHAP provides hospice accreditation to Medicare-certified programs that meet their standards of excellence.

Level 5 Partner with the We Honor Veterans Program

National Alliance for Grieving Children (NAGC)
