Make a Heartfelt Tribute

We invite you to consider making a gift in honor of a loved one in support of Care Dimensions. Care Dimensions focuses on improving the quality of life for our patients and their families. We treat patients with dignity and respect, so they may live as fully as possible. That is our legacy and we hope it will become your legacy as well.

There are several ways you can show your appreciation for Care Dimensions while honoring your loved one. You can make a memorial gift of any level at any time. Or, if you are looking for a more permanent tribute, there are naming opportunities available at the Care Dimensions Hospice House or the Kaplan Family Hospice House, including:

• Dedicating a brick in the Garden of Remembrance (minimum gift level is $1,500). Tribute bricks, inscribed with loved ones’ names, line the walkways of the Garden of Remembrance at both hospice houses. With their beautiful landscaping, your tribute bricks and the gardens add a lovely dimension to the peaceful surroundings.
• Making an inscription on the Donor Wall (minimum gift level is $10,000).
• Naming a room or location at one of our hospice houses (minimum gift level is $15,000).

Regardless of how or at what level you decide to support Care Dimensions, we will work with you to ensure your commitment honors your loved one in a meaningful way.
