Honor a Loved One’s Memory

We invite you to make a gift to Care Dimensions to honor the memory of a beloved family member, friend, colleague or community member. Your gift honors your loved one and supports other families, ensuring the comfort, caring and solace that Care Dimensions’ staff brings is available to all.

There are many ways you can make a memorial gift

• Make a donation by check payable to Care Dimensions and mail to 75 Sylvan Street, Suite B-102, Danvers, MA 01923. Please indicate the name of the person you wish to honor.
• Donate online by using our secure online service to charge your donation to your credit card.
• Download our form and mail with your check or credit card information.
• Ask if your employer has a Matching Gift Program that can double or even triple your gift. Check with your human resources department for guidelines.

Obituaries

Many families wish to designate gifts directly to Care Dimensions after the death of a loved one.

Sample language:
“Memorial donations (or The family requests memorial donations) be made to Care Dimensions, 75 Sylvan Street, Suite B-102, Danvers, MA 01923 or online at CareDimensions.org/Donate."
