Since 1978, Care Dimensions has been a driving force in expanding access to hospice and palliative care in Massachusetts. In 2015, we received the prestigious Circle of Life Award from the American Hospital Association and other healthcare organizations for being a national leader in palliative and end-of-life care. Today, Care Dimensions is the largest hospice in Massachusetts and we offer a continuum of clinical care for adult and pediatric patients with advanced and life-limiting illnesses and provide support for their families and caregivers.
Care Dimensions serves people of all ages, races, faiths, gender identity, sexual orientation, ethnic backgrounds and socio-economic standing and is committed to helping more individuals and families access care and receive support during one of the most challenging times in their lives.
By considering every facet of a patient’s life, members of the Care Dimensions clinical team bring a holistic perspective to the care they give. Each patient is viewed as a whole person with physical, emotional, spiritual and social needs. Patients and their loved ones find comfort and relief in Care Dimensions' integrative and individualized approach. We offer specialized clinical programs for patients with advanced dementia, cardiac and respiratory diseases. In addition, we provide pediatric hospice and palliative care, and programs tailored to the needs of veterans and adults with developmental disabilities.
We provide care for patients wherever they call “home” – private residences, assisted living facilities, long-term care facilities, hospitals, group homes and veteran’s facilities. Patients who need acute symptom management can be admitted to our 20-bed Kaplan Family Hospice House in Danvers, which opened in 2005 as the first licensed inpatient hospice facility in Massachusetts, or our 18-bed Care Dimensions Hospice House in Lincoln, which opened in 2018.
For patients who aren’t ready for hospice, our palliative care program adds a layer of support for patients with serious illness by providing relief from pain and other symptoms; assisting families in making difficult medical decisions; coordinating care with the patient’s physicians; and providing emotional and spiritual support.
Care Dimensions is dedicated to educating the communities we serve, the professionals that care for our patients and families, as well as the next generation of physicians, nurses, social workers and spiritual counselors. As a clinical training site for the Harvard Interprofessional Palliative Care Fellowship, Care Dimensions is proud to provide education, mentoring and in-depth experience with the full hospice interdisciplinary care team to 12 physician fellows every year. Additionally, we provide valuable experience for pharmacy interns, medical students, nursing students and interns in social work, spiritual counseling, bereavement, music therapy and child life.
The Care Dimensions Learning Institute provides outstanding training and accredited continuing education opportunities to more than 5,000 healthcare professionals every year. We also offer workshops and classes to members of the community who seek to better understand issues surrounding advanced illnesses and end-of-life decisions.
Care Dimensions is widely recognized for its bereavement program – considered to be the most comprehensive in New England. We help honor the memory of those lost by providing grief support to more than 5,000 adults and children each year. We offer counseling services for family members of patients who used our hospice service, plus workshops, support groups, and a child bereavement camp.
